Registered Nurse, seven (7) to ten (10) years long-term care nursing experience with Registered Nurse license required; or equivalent combination of education and experience, MSN preferred. Incumbent should be able to analyze information received and lead staff to achieve positive outcomes for residents, possess high-quality verbal and written communication ability, strong interpersonal and leadership skills, and be knowledgeable of nursing principles and state and federal health regulations. Must be proficient in the operation of computer applications.

This position is primarily responsible directing the overall operations of the Nursing Department in accordance with current state and federal regulations. Additional duties include overseeing scheduling of staff, budget proposal, and supervisory roles.

Key Responsibilities:

Supervises the delivery of all nursing operations and functions of Health Center on a daily basis and ensures effective and efficient resident care.

Assures the Nursing Department complies with all state, federal and OSHA health regulations/standards on a daily basis.

Acts as a leader and manager of assigned positions; ensuring productivity, efficiency and accuracy in work performed.

Maintains professional presence when representing Westminster Lake Ridge; exhibits degree of professionalism consistent with standards in performance, behavior, and appearance.

Essential Duties:

  • Complies with all CMS guidelines for the completion of the Minimum Data Set.
  • Monitors changes in the Minimum Data Sets and inputs changes of data in to the computer. Reports changes to Director of Nursing as appropriate.
  • Works with the Care Plan team to develop individualized care plans for all residents of the Westminster community.
  • Evaluates each resident's condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment.
  • Inputs MDS, reentry, and discharge tracking data in to the computer in an accurate and efficient manner.
  • Completes the required Medicare assessments on a 5, 14, 30, 60 and 90 day schedule and at change of status, annual reviews. Reviews Medicare assessments on a quarterly basis or as required by the Director of Nursing.
  • Works with the Care Plan team, as the leader, in developing a comprehensive resident assessment and care plan for each resident.

Knowledge Base:

  • Incumbent should be able to apply common sense understanding to carry out instructions furnished in written and oral form, possess high-quality verbal and written communication ability, strong customer service and interpersonal skills. Must be proficient in the use of various computer applications and be knowledgeable on all state and federal health regulations.


Bachelor's degree in Nursing, one (1) to three (3) years of long term care MDS coordination and care planning experience with a valid Registered Nurse license required; or equivalent combination
