Westminster at Lake Ridge is a premier Continuing Care Retirement Community serving older adults in three levels of service: Independent Living, Assisted Living and a Health Center. Our team of roughly 270 total employees serves a population of over 400 total residents on 62 acres that back up to the Occoquan reservoir.

We are currently seeking a compassionate LPN to join our Memory Support Program on a full time basis. We rely on our LPN's to provide quality nursing care to our residents.

Duties include:

  1. Administer medication and treatment to residents as prescribed by care team.
  2. Fills physician orders on a daily basis.
  3. Understands and participates in resident care treatment plans to include medication, therapy and testing.
  4. Maintains the resident's chart with updated information and ensures data is accurate.
  5. Notifies physician and family members of any significant changes in resident's health or care plan status.
  6. Maintains all medication inventory, ensuring appropriate amount is on hand and expiry dates are regularly checked. Narcotic count is conducted at the beginning and end of each shift.
  7. Creates and distributes resident care and staff reports to the DON and appropriate staff.
  8. Assists with planning and/or leading individual and group programs/events for the Memory Support residents ensuring they represent at least one of the seven areas of wellness.
  9. Ensures all nurses are properly trained to perform duties and are attending all mandatory training and meetings.
  10. Monitors staffing needs and reports any requirements in a timely manner.


  1. Bachelor's degree in Nursing
  2. At least one (1) year long term care nursing experience
  3. An active/current Licensed Practical Nurse license in Virginia
  4. Must have advanced knowledge in field of practice

As a requirement of this position, the candidate should be willing to:

  • Receive annual CPR and first aid training as well as body mechanics and wheelchair safety
  • Complete CDP training and receive certification
  • Complete one hour of dementia and recreational therapy training annually
  • Participate in in-service training specifically towards Memory Support training
  • Maintain all necessary certifications
  • Keep updated on federal, state and other regulations related to the position
